More info: Output connector is pins 3-5-7-9-11 of 12-pin male shrouded externally-keyed connector (outer length 1.28", outer width 0.165"), from pins 1-2-4-C-8 of switch respectively; pins are 0.025" sq. on 0.1" centers. Switch body is 0.698" wide x 1.2" tall; bezel is 0.8"w x 1.3"h. Digits are 0.125" tall. Switch contacts are normally-closed, so all 4 outputs are connected to C (common) at switch position "0".
